How do we treat fatigue?
Fatigue can have many different causes. For Dr Heinz Lüscher, the following are the main ones:
1. Iron deficiency: many people suffer from iron deficiency. This can be easily detected in the laboratory, but is by no means the only reason for fatigue.
2. Thyroid gland: This regulates the metabolism. Both hypothyroidism and hyperthyroidism can lead to fatigue. Thyroid dysfunction is also comparatively easy to diagnose.
3. Psychological factors: Anyone who is exposed to stressful situations and is under pressure for years at work or in their private life will react at some point with chronic fatigue or even burnout. Corresponding signals should be taken seriously and good self-care is important.
4. Hormone deficiency: Hormone deficiency is also more common than you think. If someone is tired, their cortisol and DHEA levels may be too low. These can be measured in saliva and treated with bioidentical hormone creams.
5. Irritable bowel syndrome: If the bowel does not function properly, vital substances cannot be absorbed properly. The body is undersupplied and becomes tired in the long run.
If all these factors are clarified, it should be possible in most cases to get to the bottom of an existing fatigue and – with appropriate therapy of the causes – also to achieve an improvement.
It is more difficult with so-called chronic fatigue syndrome (CFS). This is when severe fatigue has been present for at least 6 months, the onset of which can be determined. The fatigue is not the result of a previous organic disease or a persistent permanent overload. Even rest and recovery periods do not bring any noticeable improvement. Finally, the quality of life is also severely impaired by the exhaustion (school, work, social life, leisure time, etc.), an obvious criterion that is usually fulfilled. The possibilities to detect CFS are limited and it can often only be diagnosed by exclusion. Treatment needs patience and is not successful in every case.

How do we treat burnout?
Burnout and depression are often mistaken for the same illness. The German word for burnout is exhaustion depression, which is confusing. Burnout is not depression, but an exhaustion of the adrenal glands due to prolonged stress. In burnout, the adrenal glands eventually produce so little cortisol that normal everyday life can no longer be managed. You are massively exhausted both physically and psychologically. In contrast to burnout, the majority of patients with depression have consistently elevated cortisol levels. In patients with burnout, however, a clear cortisol deficiency can usually be detected.
The adrenal glands sit on top of the kidneys like caps. They are two small glands that are of enormous importance for the entire metabolism. They produce several important hormones, including the so-called stress hormones cortisol, adrenaline and noradrenaline. However, the term stress hormones should not be misunderstood, because these are not only produced by the adrenal glands under stressful conditions, but are necessary for the body at all times to maintain vital bodily functions. In the case of burnout, this then also leads to the fact that, on the one hand, one is massively limited in coping with stress adequately and, on the other hand, one’s overall physical vitality is greatly reduced. Permanent stress means chronic overuse of the adrenal glands. Cortisol is the hormone that the adrenal glands produce in large quantities during long-term stress, but then literally burn out at some point, resulting in underproduction.
Triggering factors for adrenal exhaustion and burnout can be various stress factors, usually several causes together lead to burnout. However, it is always long-term, negative stress (distress), be it psychological, physical, sensory or metabolic stress.
Can burnout be avoided?
Yes, it is possible – but it needs concrete, courageous steps. Are you ready for that? There are clear signs of burnout that you can recognise before it is too late:
1. severe exhaustion
2. severe sleep disturbances
3. depression – anxiety – panic attacks
4. physical complaints
It is highly recommended to seek professional help in these cases. A cortisol saliva test (daily profile) shows the cortisol levels at different times of the day and can confirm a diagnosis of burnout in addition to a thorough anamnesis discussion.
